Topical Pain Relief for Episiotomies/Vaginal Tears | Print |  E-mail

 

An episiotomy is a surgical incision made in the perineum, the area  between the vagina and anus. Episiotomies are done during the second stage of labor to expand the opening of the vagina to prevent tearing of the area during the delivery of the baby.

The area of the episiotomy may be uncomfortable or even painful for several days. Several practices can relieve some of the pain. Cold packs can be applied to the perineal area to reduce swelling and discomfort. Use of a sitz bath can ease the discomfort. A squirt bottle with water can be used to clean the area after urination or defecation rather than wiping with tissue.

For women who experience prolonged or severe discomfort a  compounded topical preparation is often prescribed. These medications provide relief for the individual patient depending on her needs. Some patients who experience discomfort such as mild pain and itching from stitches may only need a topical anesthetic, while others who are having difficulty healing due to trauma and/or infection may require a  prescription that contains an anesthetic along with other ingredients to promote healing and combat infection.

These compounds have the benefits of avoiding extensive systemic  absorption, which is always a consideration in a nursing mother. The medication has the goal of staying  locally in the tissue where it is needed.

Analgesic rectal suppositories are another method of pain relief  especially in drowsy patients, or when oral preparation causes gastric discomfort, nausea or vomiting. Full contact rectal suppositories are also very helpful when treating rectal tears/fissures or hemorrhoids which are also common complications from vaginal delivery. These suppositories stay in contact with tissue for approximately 5 hours and provide much needed relief very quickly. They may contain various combinations of ingredients to provide pain relief, as well as  promoting  healing.
