17-Hydroxyprogesterone Caproate (17P) is a naturally occurring metabolite of progesterone that is produced in significant quantities during pregnancy. Progesterone and its analogs have been studied for use in PreTerm Delivery (PTD) since the early 1950s; however, most trials have found only 17P and progesterone to be clinically effective.

17P, via intramuscular injections, has been shown in a number of studies to significantly reduce the incidence of PTD in high-risk patients. The 2003 New England Journal of Medicine article published results from a multicentered, double-blind, placebo-controlled trial involving 463 women with a history of spontaneous PTD who were considered at high risk for further PTD. This research was the most promising to date and showed unequivocally the ability of 17P to significantly reduce the risk of PTD. **

Most literature supports the prophylactic use of 17P for women considered at high risk because of a history of spontaneous PTD after spontaneous preterm labor or premature rupture of the fetal membrane. Therapy should be initiated at 16 weeks of gestation byt no later that 24 weeks of gestation.**
