Topical nitrogen mustard (NM or Mustargen) has been used as primary therapy for management of patients with mycosis fungoides (MF) since the 1950's. Many investigators have demonstrated the efficacy of topical Mustargen in patch and/or plaque disease of MF. A study performed at Stanford School of Medicine also confirms the clinical efficacy and safety of topical Mustargen.
In the Stanford study, a total of 203 patients with mycosis fungoides were treated with topical Mustargen as initial therapy. The overall response rate for the 203 patients was 83%, with a complete response rate of 50%. The median time to achieve complete response was 12 months, and the duration of complete response increased with longer maintenance therapy.
Topical Mustargen can be used as an aqueous or ointment-based preparation. The ointment base is usually preferred due to the lower incidence of contact dermatitis (fewer than 10% in the Stanford study), stability of the formulation, and additional moisturizing benefits.
The authors of the Stanford study concluded that "Topical nitrogen mustard remains an effective primary initial or salvage therapy in mycosis fungoides. Long-term follow-up results confirm its safety."* It has also been used safely in pediatric patients and there is no evidence of any clinically significant systemic absorption of topically applied Mustargen. **
